Ozempic, Wegovy and Zepbound Pens and Needles: How to Dispose of Them and How to Travel With Them

Which needles each pen's label calls for, how to get rid of used pens and needles in the US (FDA sharps rules, state options) and how to get through TSA with your pen.

Used pen needles and single-dose pens go into a sharps disposal container right away, never loose into the trash or the recycling. When the container is about three-quarters full, you get rid of it the way your state or local community says, because the rules vary from place to place. On a trip, your pen travels in your carry-on, you declare it at the TSA checkpoint, and it never freezes.

In 30 seconds

  • Ozempic and Wegovy FlexTouch pens come with NovoFine Plus 32G 4 mm needles; their instructions allow compatible needles up to 8 mm. Zepbound and Mounjaro KwikPens come without needles.
  • Single-dose pens (Wegovy, Zepbound, Mounjaro) have the needle built in: the whole pen is the sharp.
  • A new needle for every injection. Never share a pen, even with a new needle.
  • The FDA recommends two steps: a sharps container right after use, then your community’s disposal route when it’s about 3/4 full.
  • TSA allows unused syringes with injectable medication and used ones in a sharps or hard-sided container. Declare them at the checkpoint.

Which needles each pen takes, per the label

Which length is right for you is your healthcare professional’s call; the label only sets the limits. Your pharmacist can tell you which pen needles fit your pen.

A new needle every time, and never shared

The Ozempic instructions say “Do not reuse or share your needles with other people. You may give other people a serious infection, or get a serious infection from them”. The KwikPen instructions ask for a new pen needle for each injection to help prevent infections and blocked needles.

Pen sharing is a formal warning in the Prescribing Information, not just a tip. For Wegovy: “WEGOVY FlexTouch must never be shared between patients, even if the needle is changed. Pen-sharing poses a risk for transmission of blood-borne pathogens”. Ozempic, Zepbound and Mounjaro carry the same warning for their multi-dose pens.

The Ozempic and Wegovy FlexTouch instructions also ask you to take the needle off after every injection: “Always remove the needle from your pen”. How to do that safely is shown step by step in the Instructions for Use. Follow them as written.

How to dispose of used needles and pens in the US

Step 1: a sharps container, right away. The FDA recommends putting every used needle in a sharps disposal container immediately, and replacing it when it’s about three-quarters full, because overfilling raises the risk of needle sticks. The labels say the same. Wegovy: “Put your used WEGOVY pen and needle in an FDA-cleared sharps disposal container right away after use”.

No sharps container? The labels list what a household one needs. If you don’t have an FDA-cleared container, the Wegovy, Ozempic, Zepbound and Mounjaro instructions allow a household container that is made of a heavy-duty plastic; can be closed with a tight-fitting, puncture-resistant lid, without sharps being able to come out; upright and stable during use; leak-resistant; and properly labeled to warn of hazardous waste inside the container.

What goes where depends on the pen:

Step 2: follow your state and local rules. Every label points to the same place: “There may be state or local laws about how you should throw away used needles and syringes”. The FDA lists the options communities use:

  • drop boxes or supervised collection sites, such as doctors’ offices, hospitals, pharmacies, health departments or police and fire stations
  • household hazardous waste collection sites
  • mail-back programs for certain FDA-cleared containers, usually for a fee
  • residential special-waste pick-up services

To find out which ones exist where you live, check with your local trash service or health department. The FDA also gives a phone line for state-specific information, Safe Needle Disposal at 1-800-643-1643.

The Ozempic instructions end with two rules the other labels repeat in similar words: “Do not dispose of your used sharps disposal container in your household trash unless your community guidelines permit this. Do not recycle your used sharps disposal container.”

Travelling with your pen: fridge, heat and the airport

The cold chain in two sentences. Pens you haven’t started stay in the fridge at 36 °F to 46 °F (2 °C to 8 °C) and must never freeze. Once in use, or for single-dose pens before the cap comes off, each one can stay out of the fridge up to 86 °F (30 °C) for a limited time that depends on the model; I keep that table in the holidays guide.

If you carry spare pens in a cooler, I’d keep them from touching ice or ice packs. That’s my own inference, not a label sentence: the Wegovy label says “Do not store WEGOVY in the freezer or directly beside the refrigerator cooling element” because the pen can’t freeze, and ice poses the same risk. The Ozempic instructions are explicit about the car: “Do not leave it in a car or other place where it can get too hot or too cold”. The Zepbound and Mounjaro KwikPen instructions say “Keep away from heat and light”.

At the TSA checkpoint

TSA’s item pages cover syringes and liquid medications:

  • Unused syringes are allowed in carry-on bags when accompanied by injectable medication, and you must declare them at the checkpoint. TSA recommends, but doesn’t require, that your medications be labeled.
  • Used syringes are allowed in a sharps disposal container or a similar hard-surface container.
  • Liquid medications are allowed in larger amounts, in reasonable quantities for your trip, if you declare them.

On every page, the final decision rests with the TSA officer. Pen needles aren’t named separately. My reading is to treat them like syringes: declare the pen and the needles together.

Labels and a letter. The FDA’s sharps page suggests keeping your medicines labeled with the pharmacy label or the manufacturer’s name, and bringing a letter from your doctor. It also says to always carry a small, travel-size sharps container in case no other option is available. If you fly internationally, check the rules of the country you’re going to before you leave.

If your pen froze or got too hot

Frozen means don’t use it. Wegovy: “Do not freeze WEGOVY and do not use WEGOVY if it has been frozen”. The Zepbound and Mounjaro KwikPen instructions: “Do not freeze your Pen. Throw away (discard) the Pen if it has been frozen”. That holds even if it has thawed and looks normal.

Above 86 °F (30 °C). The Wegovy labels are the most direct. Single-dose pen: “Throw away the pen if WEGOVY has been frozen, has been exposed to light or temperatures above 86°F (30°C), or has been out of the refrigerator for 28 days or longer”. The FlexTouch pen has the same rule with 56 days or longer out of the refrigerator. The Zepbound and Mounjaro labels set 86 °F (30 °C) as the storage limit but don’t say what to do if it was exceeded.

Out of the fridge too long. Zepbound single-dose pens and vials: up to 86 °F (30 °C) for a total of up to 21 days, then throw them away. KwikPens: throw away after a total of 30 days at room temperature, 30 days after first use, or 4 weekly doses.

If a pen got hot and you’re not sure about it, ask your pharmacist before your next injection. If you have to throw away a pen with doses left, talk to your prescriber or pharmacist about how to continue. You can note the question in the doctor-visit checklist.

Frequently asked questions

Why doesn’t my Zepbound KwikPen come with needles, and which ones do I buy?

The label lists a KwikPen-compatible pen needle as “not included”. Ask your prescriber or pharmacist which compatible needle to use; the pharmacy can tell you what fits.

Can I use 4 mm needles or do they have to be 8 mm?

For Ozempic and Wegovy FlexTouch, the instructions come with NovoFine Plus 32G 4 mm needles and allow NovoFine Plus, NovoFine or other compatible needles up to 8 mm. Which length you use is your healthcare professional’s decision.

How do I dispose of Zepbound or Ozempic pens?

It depends on the pen. A Zepbound or Mounjaro KwikPen can go in household trash once the needle is removed. Single-dose pens and empty Ozempic or Wegovy pens go into a sharps container. Loose needles never go in the trash.

Can I reuse the needle if I’m the only one using it?

No. The labels ask for a new needle for every injection, to prevent infections and blocked needles, and say never to share a pen, even after changing the needle.

Can I pack my pen in my checked bag?

TSA allows medications and unused syringes in both carry-on and checked bags. Every label says the pen must never freeze, and in your carry-on you know where it is and what temperature it’s at. TSA asks you to declare liquid medications and unused syringes; it doesn’t name pen needles, so my reading is to declare the pen and the needles together.
