How to fill it in

Take a minute at the end of the day. Rate each symptom from 0 to 3 and leave blank anything you didn’t track: a blank is not the same as a 0. In the notes, write down whatever helps explain the day: what you ate, when the symptoms started, and what made them better or worse, which is what MedlinePlus suggests describing at your appointment.

Intensity scale for each symptom

Appetite: from 0 (not hungry at all) to 10 (very hungry). Fluids: glasses of about 8 fl oz (250 ml). Protein: an estimate in grams. Dose taken: the dose you took as your prescriber set it, or blank. The scale is for comparing your days with each other: a 3 is not a warning sign on its own, and having no 3s doesn’t rule one out. That is what the box above is for.

How I made this diary

The eight symptoms in the diary are among the most common side effects listed in the Wegovy Medication Guide: nausea, vomiting, diarrhea, constipation, stomach pain, tiredness, dizziness, and heartburn (the reflux row). Appetite, fluids, and protein help show how you are eating. The 0 to 3 scale is a simple way to compare your days; it is not a validated clinical scale and it can’t be used to diagnose anything.

The warning signs are quoted word for word from the US Medication Guides for Wegovy and Zepbound, the versions for people in the United States. The Spanish version of this diary (in Spanish) quotes the European patient leaflets for Wegovy and Mounjaro instead; otherwise the two diaries are the same. The diary doesn’t interpret what you log or tell you what to do: it keeps your notes so you can go over them with your prescriber.

Your entries are saved in your browser’s local storage. I don’t receive or see them, they don’t sync between devices, and they disappear if you clear the site’s data or press “Delete everything”. The English and Spanish pages share the same storage, so a diary started in one language shows up in the other. To keep a copy, export the CSV or save the PDF.
